1. What is ultrasonic rhinoplasty?

Ultrasonic rhinoplasty is the most advanced technique in rhinoplasty surgery. It is known and used by very few surgeons worldwide, and Dr. Pablo Casas is one of them. This method makes it possible to reshape the nasal bones without breaking them and in a less traumatic way, since it does not damage the soft tissue or the blood vessels covering the bone—reducing the swelling and bruising caused by the traditional method.

Ultrasonic instruments allow Dr. Casas to perform a more controlled and precise reshaping of the nasal bones, providing greater control over the surgical outcome.

2. Besides having breathing problems, I don’t like the shape of my nose. Can everything be corrected in the same procedure?

Yes. If a person has a deviated septum that affects breathing function, and a nasal shape they do not like, the ideal time to correct both is in the same surgery.

The best results are achieved when both aspects are corrected in a single procedure. Functional problems and aesthetic concerns can be addressed at the same time. A deviated nasal septum can cause breathing problems that can be corrected with a septoplasty. When rhinoplasty and septoplasty are combined to improve both the aesthetic and functional aspects of the nose, it is called septorhinoplasty.

4. From what age can I undergo rhinoplasty?

Rhinoplasty should be performed once facial growth and development have finished—meaning from 17–18 years of age onwards.

5. What type of anesthesia is used?

Rhinoplasty is performed under general anesthesia, with the patient fully monitored. Dr. Casas operates with top-level anesthesiologists. From our point of view, rhinoplasty should NEVER be performed under local anesthesia, as it is less safe.<br
Today, general anesthesia is, in all likelihood, the safest medical procedure. Safer than traveling by car, train, plane, or other daily activities.

Rhinoplasty surgery should ALWAYS be carried out under the strict supervision of an anesthesiologist throughout the entire operation. The anesthesiologist must monitor heart rate, respiratory function, temperature, and fluid and electrolyte replacement.

Rhinoplasty surgery lasts 2–3 hours, and performing it under local anesthesia without an anesthesiologist present is imprudent. Injecting local anesthesia into the nose requires multiple needle punctures, which is uncomfortable. Remaining still for two hours, feeling blood trickle toward the throat, and watching a surgeon operate is not pleasant. It is false that local anesthesia is risk-free.

The medications used for local anesthesia can cause cardiac arrhythmias, as can the patient’s stress if they remain awake during surgery.

It is not advisable to perform rhinoplasty using local anesthesia alone without an anesthesiologist present. Not having an anesthesiologist in the operating room is unsafe.

7. Does rhinoplasty surgery hurt?

This is a very common question, and Dr. Casas always says the same thing: NO.

Rhinoplasty does not hurt. Recovery is fairly quick, and within a few days you can return to almost normal activity. The main symptoms are nasal congestion similar to a cold, which gradually improves over about 6 to 7 days. This congestion can cause a mild headache sensation, which typically resolves easily with a mild painkiller.

8. When can I return to my usual routine or go back to work?

After surgery, you will need to wear a nasal splint for one week. Patients typically return to work 7–10 days after surgery, as by then bruising under the eyes and most of the swelling will have disappeared.

Even so, the nose will continue its de-swelling process over the following weeks. Intense sports should be avoided during the first month.

Around 70% of post-rhinoplasty swelling decreases during the first month. The remaining 30% can take 9 months to a year to fully subside.

9. When can I fly after rhinoplasty surgery?

Once the nasal splint is removed 7 days after surgery.

10. I’ve hit my nose—what should I do?

First, consult an expert in nasal surgery. It will be necessary to evaluate the internal nasal structure to confirm whether breathing function has been affected, and also assess the consequences on the external appearance to determine how and when it can be corrected.

11. I am a man—am I also a candidate for rhinoplasty?

Traditionally, rhinoplasty was considered something only for women. Times have changed, and today men are also interested in improving their appearance, understanding that a better look results in more confidence and greater opportunities for success in both professional and personal life.

Men often come to consultation due to a prominent nasal hump, a drooping tip, or a crooked nose caused by trauma, which is more frequent in young men.

12. Can rhinoplasty improve snoring?

Rhinoplasty surgery improves the external appearance of the nose and helps the patient breathe better. By improving nasal breathing, the patient will stop breathing through the mouth, and snoring will improve.
